Off-white bakelite double switch cover made by Sierra Electric Corp. It has a  vertical stripe design along the middle section. The cover measures 4 1/2″l x 4 1/2″h. It is attached by 4 screws (not included). The plate is marked on the back Sierra Electric, Gardena Calif. Made in U.S.A..
